Your MRI Is Probably Scarier Than Your Back
Every week, a patient sits across from me holding an MRI report like a verdict. Degeneration. Bulge. Desiccation. The words sound catastrophic. The evidence says otherwise.
What scans find in people with no pain
Systematic reviews of asymptomatic adults — people with zero back pain — show disc degeneration in 37% of 20-year-olds and 96% of 80-year-olds. Disc bulges appear in roughly a third of pain-free young adults. These findings are so common that many researchers now call them 'wrinkles on the inside': normal features of a spine that has been used.
When imaging genuinely matters
Imaging is essential when red flags are present: significant trauma, unexplained weight loss, fever, progressive neurological deficit, or loss of bladder and bowel control. Outside those scenarios, international guidelines consistently recommend against routine imaging for back pain — because it doesn't improve outcomes, and the frightening language often makes them worse.
What actually predicts recovery
Your prognosis is better predicted by sleep quality, activity levels, stress, and — remarkably — your own beliefs about your back than by anything on the scan. Patients who understand their spine is strong and adaptable consistently recover faster. That's not positive thinking; it's one of the most replicated findings in pain science.
If you've been handed a scary report, bring it to a clinician who will translate it honestly. Your spine is almost certainly more capable than the radiology jargon suggests.
